Two forms of ID required; one must be a photo ID, one must bear your signature. Examples of acceptable IDs include: valid driver’s license, passport, credit card with a signature.

If your document requires a witness, you must bring someone you know with you to the appointment. Library staff cannot serve as witnesses.
